April Focus: Community Health Coalition Champions Collaborative Care for Holistic Healing
DURHAM, NC — This April, the Community Health Coalition (CHC) is highlighting Collaborative Care as part of its monthly Health Area Focus campaign. The goal? To improve access to coordinated, whole-person care that treats both the mind and body—leading to better health outcomes and breaking down barriers to effective treatment.
What Is Collaborative Care?
